1. Acceptance of Terms

By accessing the AF Dental Smile LLC website or using our dental laboratory services, you agree to be bound by these Terms of Use. If you do not agree to these terms, please do not use our website or services.

2. Services Description

AF Dental Smile LLC is a full-service digital dental laboratory providing dental restorations and related services, including but not limited to digital design (CAD/CAM), digital milling, digital printing, crowns, bridges, veneers, inlays/onlays, and implant restorations. All services are provided exclusively to licensed dental professionals.

3. Payment and Terms Agreement

AF Dental Smile LLC is dedicated to provide the highest quality products to our customers. Your partnership and trust in our services is highly appreciated.

The following Payment and Terms Agreement is our set standard which we have for all our clients. By completion of this form, you have agreed to provide us with your payment method of choice and understand the payment terms as provided. AF Dental Smile LLC will send an invoice with every case, which details the products and costs you received. In addition, a monthly statement will be mailed to you which includes all invoices and balances owed. We are a small business and highly appreciate prompt payment. The payment is due within 15 days of the receipt of the monthly statement. If the payment is not received, a 2% monthly finance charge will be applied to the account balance. Any account with a balance 45 days past due will be placed on hold, until the balance is paid in full. You can pay by phone 412-616-1515 or conveniently per payment method of your choice.

**The person signing this form is an authorized signer and, along with the dental practice, accepts responsibility for payment of all related charges, as well as any legal costs, collection and other fees incurred by AF Dental Smile LLC in the event the account is sent to collections or litigation.

4. Warranty and Remake Policy

AF Dental Smile LLC stands behind the quality of our work. If a restoration does not meet the agreed-upon specifications or has a manufacturing defect, we will remake the case at no additional charge. Remakes must be requested within 30 days of delivery. The warranty does not cover damage caused by improper handling, adjustment, or clinical use after delivery. Cases must be returned to our lab for evaluation before a remake is authorized.

5. Turnaround Times

Standard case turnaround time is 8–10 business days from the date we receive the case or digital files. Rush cases are available upon request and may be subject to additional fees. Turnaround times are estimates and not guaranteed delivery dates. AF Dental Smile LLC is not liable for delays caused by shipping carriers, incomplete case information, or circumstances beyond our control.

6. Shipping and Delivery

Completed cases are shipped via standard carrier. Shipping costs are the responsibility of the client unless otherwise agreed upon. AF Dental Smile LLC is not responsible for loss or damage during transit. We recommend using trackable shipping methods when sending cases to our lab.

7. Cancellation and Refund Policy

Cases may be cancelled before production begins at no charge. Once production has started, cancellation fees may apply based on the stage of completion. Refunds for completed work are issued at the sole discretion of AF Dental Smile LLC and only after the case has been returned and evaluated.

8. Limitation of Liability

AF Dental Smile LLC provides dental laboratory products to licensed dental professionals only. We are not responsible for clinical outcomes, patient reactions, or any damages arising from the improper use, modification, or placement of our restorations. Our liability is limited to the cost of the case in question. In no event shall AF Dental Smile LLC be liable for indirect, incidental, or consequential damages.

9. Intellectual Property

All digital designs, CAD files, and proprietary processes created by AF Dental Smile LLC remain our intellectual property. Clients retain ownership of their submitted scans and case information. Digital designs may not be reproduced, shared, or used by third parties without our written consent.

10. Confidentiality and HIPAA

AF Dental Smile LLC is committed to protecting the confidentiality of all patient and practice information. We comply with HIPAA regulations regarding the handling of protected health information (PHI). All digital files, patient data, and case records are stored securely and accessed only by authorized personnel on a need-to-know basis.

11. Website Use

You may use our website for informational purposes and to access our services. You agree not to misuse the website, attempt to gain unauthorized access to our systems, or use the site for any unlawful purpose. We reserve the right to restrict access to any user who violates these terms.

12. Changes to Terms

AF Dental Smile LLC reserves the right to update or modify these Terms of Use at any time without prior notice. Changes will be posted on this page with an updated revision date. Your continued use of our website or services after changes are posted constitutes your acceptance of the revised terms.

13. Governing Law

These Terms of Use shall be governed by and construed in accordance with the laws of the Commonwealth of Pennsylvania. Any disputes arising from these terms or our services shall be resolved in the courts of Allegheny County, Pennsylvania.
